Attending a large public event? Plan ahead with these pro tips

  2. Create an account using your Colorado driver license or state-issued ID . This will generate your Colorado Digital ID ™ , which can be used for proof of ID, age and address within the state.
  3. Access myVaccine Record and download a copy of your digital COVID-19 vaccination record to your device. This ensures you have access in multiple places on your device if you ever experience connectivity issues. iOS users: See the steps below on how to add your Colorado SMART® Health Card to the Apple Wallet.

  1. Tap the COVID-19 Vaccination Record button on the Wallet screen to view your Colorado SMART® Health Card.
  2. Tap the green arrow at the top of the screen to download your unique QR code.
  3. Choose Save Image .
Continue with the steps below to add your SMART® Health Card to the Apple Wallet
  1. Open Photos and find your Colorado SMART® Health Card QR code.
  2. Press your finger on the QR code image and hold until the Open in Health button appears.
  3. Tap the Open in Health button. You may need to follow additional prompts to set up your Health details if you haven't already.
  4. Confirm your information is correct and tap Add to Wallet & Health .
  5. You will see a Vaccination Added confirmation screen. Tap Done.

COVID-19 Vaccination Record Record FAQs

How soon after receiving a COVID-19 vaccine, including a booster, can I access myVaccine record?

After you receive your vaccine, your healthcare provider should promptly report your vaccination to the Colorado Immunization Information System (CIIS). This time may vary, so please allow 24-72 hours for your vaccination to appear in COVID-19 Vaccination Record .

H ow do I refresh COVID-19 Vaccination Record after receiving my second or third vaccine dose?

O pen COVID-19 Vaccination Record and tap the green Update COVID-19 Vaccination Record button. Enter either your phone number or email address and tap Submit .

I received more than three COVID-19 vaccine doses, will this be visible in COVID-19 Vaccination Record?

Yes. With the new scrolling feature, all COVID-19 vaccinations and boosters will be visible on the vaccination record card in the app.

Can I access my vaccine card in COVID-19 Vaccination Record if I have not been fully vaccinated?

Yes. Even if you are partially vaccinated, you can still access myVaccine Record and your vaccine status will show as “Partially Vaccinated for COVID-19.”

My contact information was recently updated in CIIS. When can I expect to be able to access my record?

It will take a minimum of 24 hours for your updated contact information to reflect on your account.

Can I access my children’s vaccine record?

No. You are only able to access your own personal record.

Can I access other immunization records besides my COVID-19 vaccination information?

No. You are only able to access your COVID-19 vaccination record.

I was vaccinated in another state, how do I report this to Colorado so I can access COVID-19 Vaccination Record in the myColorado app?

If you are a Colorado resident and have a Colorado driver license or state-issued ID, you can call your provider in the state you received your vaccine and ask them to report your vaccination record via email to CIIS at cdphe.ciis@state.co.us. As soon as your vaccination information has been reported to CIIS, you can pull your own COVID-19 Vaccination Record in the myColorado app. At this time, only Colorado residents with a Colorado driver license or state-issued ID can use myColorado and COVID-19 Vaccination Record .

Will my immunization information remain private?

Yes! Use of the myColorado app to retrieve vaccination records is voluntary on an opt-in basis. No data will be accessed or shared except that which is requested by a myColorado user who has been authenticated through facial recognition. A retrieved vaccination record is only visible to the myColorado user who downloaded it, and the data is not shared with anyone else in the retrieval process.

Is this a vaccine passport?

No. The COVID-19 Vaccination Record is a convenient digital version of your paper vaccination record card that draws your immunization information directly from Colorado’s Immunization Information System (CIIS) for conven ient and easy access .

Are there other ways for me to obtain a copy of my COVID-19 vaccine record?

Yes. In addition to the paper CDC COVID-19 Vaccination Record Card you were given when you received your COVID-19 vaccine(s) and COVID-19 Vaccination Record , you can obtain a copy of your COVID-19 vaccine record via the Colorado Immunization Information System (CIIS) or c ontacting your primary care provider.

I don’t want my immunization record card displayed anymore. Can I delete it?

Yes. Click the T rash C an icon in the upper righthand corner and follow the prompts.

What is a QR code?

A QR code (or "quick response" code) is a type of barcode that contains certain information that can read or interpreted with smart devices. The information is encrypted, providing an extra layer of security.

Does the Colorado SMART® Health Card replace COVID-19 Vaccination Record ?

No. The Colorado SMART® Health Card is a new option within your myVaccine Record.

What information is revealed in the Colorado SMART® Health Card Verifier App?

Only key information needed for verification is shown when your Colorado SMART® Health Card is scanned

Is my data stored by the Colorado SMART® Health Card Verifier App?

No personal data is collected about you as the user of the app. While the app will display some personal data about the person showing their Colorado SMART Health Card to verify their COVID-19 vaccination status, no personally identifiable information scanned from the Colorado SMART Health Cards will be shared .

Can the Verifier app scan my CDC card?

The Verifier app only scans SMART® Health Cards. Printed vaccination records that do not include a QR code, including CDC cards, cannot be scanned by the app.

Is there a fee to use a SMART® Health Card?

No. The myColorado™ app will continue to be free and there is no fee to get or use a Colorado SMART® Health Card. If you decide to share your Colorado SMART® Health Card with someone, there should be no fee for you or them.

Will my Colorado SMART® Health Card be accepted everywhere that asks for my COVID-19 vaccination or test status?

It depends. Though Colorado SMART® Health Cards are designed so they may be used by any organization that wants to use them, there is no requirement that they do so.

I don’t wish to use my Colorado SMART® Health Card. How do I opt out?

The Colorado SMART® Health Card is part of myVaccine record, however, you do not need to use it. Choosing to share your information is always in your control.

Can the Colorado SMART® Health Card be shared without my permission?

No. Your passcode history and/or test results are stored directly within the Colorado SMART® Health Card as a 2D barcode (QR code) or file that you control. Sharing your card is completely up to you.

What happens to my information after I share it?

Sharing your Colorado SMART® Health Card information is just like sharing a paper copy of immunization records with a school, faxing medical records to a physician’s office or using your credit card at a restaurant. You should only share your SMART® Health Card information with trusted organizations that tell you what they intend to do with your data and/or if they will keep it. If you are not comfortable with the organization seeing the information in your SMART® Health Card, you should not share it.

Can someone steal my Colorado SMART® Health Card?

myColorado uses multi-factor authentication to help keep your account secure by requiring account credentials along with additional information e.g., a passcode sent to your phone. Your SMART® Health Card can be presented by anyone holding it. However, the Colorado SMART® Health Cards are verified with another form of identification, such as a student account in a school system or a driver’s license.

Does sharing my Colorado SMART® Health Card disclose any government-issued identifiers, like my social security number or citizenship status?

No. The card should only contain your legal name, date of birth, and clinical information like vaccination history. A Colorado SMART® Health Card issued in other countries could include commonly used identifiers, depending on local practices or regulations.

Will I be able to use my Colorado SMART® Health Card to store and share other clinical information?

Not at this time.

SMART® Health Card Verifier App - for Businesses

What is the SMART® Health Card Verifier App?

The SMART® Health Card Verifier App allows someone to scan a SMART® Health Card QR code to quickly verify an individual’s COVID-19 vaccination status. The service is provided by The Commons Project Foundation, built on standards from SMART® Health IT and powered by Affinidi.

The verifier app is designed for anyone who needs to be able to quickly verify individuals’ COVID-19 vaccination status in a secure and fraud resistant way. Free to use , the SMART® Health Card Verifier app makes verifying vaccination status easy for businesses, from bars and clubs to restaurants to sporting venues to employers.

How does the SMART® Health Card Verifier work?

When a SMART® Health Card QR code is scanned by the SMART® Health Card Verifier app, two key pieces of information are displayed: (1) whether the SMART® Health Card is valid and (2) whether the issuer is in The CommonTrust Network’s registry of trusted issuers. Other information displayed includes issuer name, vaccine type, dates of vaccine doses, and name and date of birth of the vaccine recipient.

Note: The SMART® Health Card Verifier App does not scan paper CDC cards.

I am a business or venue. How do I get and use the SMART® Health Card Verifier app to confirm the individual’s vaccination status?

What does it mean if a Colorado SMART® Health Card is partially verified?

A "Partially Vaccinated" will display if a user has one valid Moderna or one valid Pfizer vaccination.

What does it mean if a Colorado SMART® Health Card is not verified?

If a user does not have any valid COVID-19 vaccinations in their CIIS record, a Colorado SMART® Health Card will not be displayed.

Troubleshooting Tips

Why can’t I access my vaccination record via COVID-19 Vaccination Record ?

While most providers in Colorado report vaccinations to the Colorado Immunization Information System (CIIS), federal providers do not. If you received your COVID-19 vaccine from a federal provider, please contact the health care professional or clinic where you received your vaccine and ask them to report your record to CIIS.

N ote: The contact information you enter on the myColor ado app must match the contact information associated with your record in CIIS. If you have questions about your immunization record, please fill out this form to request assistance.

I received an error message stating “We didn’t find an exact match” or “More than one immunization record was found.” What should I do?

If you're having difficulty accessing your record or see an incorrect record, please complete the CDPHE Immunization Record Information Request Form . In some cases, you may need to speak with the healthcare provider who administered your vaccine to confirm they submitted your correct first, middle and last names, phone number and/or email address.

Where did my downloaded vaccine record go (Android users)?

Some LG and Motorola phones don’t come with an app that lets you open downloaded PDF documents. If you have downloaded your vaccine record but cannot locate the file on your phone, you may need to install a PDF reader app on your phone to access the file. Several free apps are available on Google Play.

Where did my downloaded vaccine record go (iOS users)?

Download the PDF and access via your Files app. Click Browse -> iCloud Drive -> myColoradoicloudcontainer folder.

If you are not able to locate your PDF, download in your Files app or you receive a “Sign into iCloud” error, please check your iOS settings. First, confirm that you are signed into your iCloud account. Once you are signed in, make sure that your iOS Wallet app is enabled by accessing your phone Settings -> Apple ID -> iCloud -> Wallet . Tap the toggle so that it is green or in the “on” position. To disable your Wallet, tap the slider again until it turns grey or in the “off” position.

COVID-19 Vaccination Record is blank or showing “null” in my personal information screen. I can’t type in the text fields to enter my information. How do I fix this?

If you are having trouble accessing your vaccination record in the Wallet , or are seeing “null” in the name fields when opening COVID-19 Vaccination Record :

  1. Update your app to the latest version from the App Store or on Google Play.
  2. Once updated, open the app. You may need to log in with your username and password.
  3. Tap Wallet and then tap COVID-19 Vaccination Record .
  4. Enter your email address or phone number and tap Submit .

You should now see your record in the app.

What if I can’t see the scroll option on the View Vaccination Card screen?

You will not see the new scrolling feature unless the number of your vaccines is more than your screen is able to display. Depending on the size of your screen up to 10+ vaccinations may be visible before the scrolling feature is triggered.

If you cannot see all your vaccinations, or see two digital vaccination cards stacked on top of one another in COVID-19 Vaccination Record, make sure the latest app version is installed and tap the green Update COVID-19 Vaccination Record button to input your phone number or email address associated with your medical record. Once this is completed you should see your full list of vaccinations.

For users who have a mini phone (e.g., iPhone mini 12 or iPhone mini 13), you will need to zoom in before you are able to activate the scroll function.

Why can’t I see all my vaccines when I import my COVID-19 vaccinations into Apple Wallet?

In Apple Health, all of your COVID-19 vaccination records can be shown. However, only the latest four vaccines are able to be imported into Apple Wallet at this time.
