Form C- Verification of Graduate Coursework: Applicants for a clinical addiction counselor associate license must have a graduate in addiction counseling, addiction therapy, or in a related area as determined by the board from an eligible postsecondary educational institution that meets the requirements of the board. All education criteria must be met by postsecondary educational courses. Certifications cannot be used to meet educational requirements.

Form P- Verification of Practicum: Applicants must complete not less than one (1) supervised practicum, internship, or field experience in a counseling setting, which must include a minimum of seven hundred (700) clock hours. The 700 hours must have a minimum of two hundred eighty (280) face-to-face client contact hours of addiction counseling service and a minimum of thirty-five (35) hours of supervision from a qualified supervisor who has at least five (5) years’ experience in addiction counseling. As used in IC 25-23.6-10.5-6, "qualified supervisor" means an individual who is supervising within his or her scope of experience and training and holds an active license at the time of supervision as:

A clinical addiction counselor;
A clinical social worker;
A marriage and family therapist;

Official Score Report: Applicants must contact the appropriate examination reporting service (for National examinations) or the State Licensing Board (for State constructed examinations) and request that an official score report. Your official score report may be either sent to the Indiana Professional Licensing Agency, or uploaded to your online account. Any examination other than the ADC or Level II will be reviewed by the Board on an individual basis to determine equivalency.

After the approval of your application by the Board, you will be emailed the law examination. You will have fourteen (14) days from the date the email is sent to you with the Jurisprudence Examination and Instructions to complete the examination and return the required information to our office. Post-Degree Experience: Applicants must have at least three thousand (3,000) hours of post-graduate clinical experience over a two (2) year period. This two (2) year period means experience under approved supervision, acquired over no less than twenty-one (21) months and over no more than forty-eight (48) months. A doctoral internship may be applied toward the supervised work experience requirement. The clinical work requirement may not be performed away from the supervising mental health counselor's premises if:

The work is the independent private practice of mental health counseling; and

The work is not performed at a place that has the supervision of a licensed mental health counselor or an equivalent supervisor, as determined by the board. As used in IC 25-23.6-8.5-4, "equivalent supervisor" means an individual who is supervising within their scope of experience and training and is any of the following:

Licensed as a clinical social worker.
Licensed as a marriage and family therapist.
Licensed as a physician who has training in psychiatric medicine.
Licensed as a psychologist.
Licensed as a clinical nurse specialist in psychiatric or mental health nursing.
A mental health professional of equivalent status if the supervision was provided in a state where no regulation exists.

Post-Degree Supervision: Applicants must also show that they have completed two (2) years of supervision acquired during their 3,000 hours of post-graduate clinical experience. This supervision must be completed in no less than 21 months, and no more than 48 months and show that they have acquired at least one hundred (100) hours of face to face supervision. Form III - Verification of Graduate Coursework: Applicants for a marriage and family associate license must have received a master’s or doctor’s degree in an area of marriage and family therapy, or in a related area as determined by the board, from an eligible postsecondary educational institution that meets the requirements of the board. All education criteria must be met by postsecondary educational courses. Certifications cannot be used to meet educational requirements.

Form P - Verification of Practicum: Applicants must complete at least one (1) supervised practicum, internship, or field experience in a marriage and family counseling setting, which must include a minimum of three hundred (300) hours. The three hundred (300) hours of marriage and family therapy services must be completed in no less than twelve (12) months of clinical practice and broken down into the following:

Form III - Clinical Coursework: This form must be completed and submitted with your application. Form II - Verification of Experience: Applicants must submit proof of two (2) years of experience in the practice of clinical social work in counseling of clients in a clinical setting. If hours are obtained in Indiana they must occur under an Active LSW license or Temporary LSW Permit. The experience must be paid, full-time and supervised to qualify as experience. Part-time experience will be considered by the Board. (Note: One thousand five hundred hours (1,500) equals one (1) year of experience.) Renewal FAQs

How many continuing education hours do I need to renew my license

40 hours of continuing education credits are required for each renewal cycle or 20 CEUs per year.
Must complete at least 20 hours of Category 1 per renewal cycle.
Must complete at 2 hours of Ethics per renewal cycle (1 hour of ethics a year)

You will need 20 hours in order to renew your license.

You do not need any continuing education in order to renew your license.

You can record your hours on a Word document or spreadsheet. You should include the date of the activity, the type of activity, and the amount of time you spent conducting the activity.

You can take as many CEUs as you like as long as you take at least 20 CEUs per year, a minimum of 10 of those hours must be Category I.

No. Credits can only be counted for the period in which they are taken.

No. Certificates are for your records and will only need to be submitted if you are audited. However, it is required that licensees maintain copies of their CEU records for a period of four years in the event they are audited by the Board.

Once you receive your audit letter you will need to either fax or mail in copies of continuing education certificates to the Board by the deadline stated on the letter.

The Board has adopted the Association of Social Work Board’s (ASWB) examination to obtain a social work license. The examination is only offered in a computer format through the Association of Social Work Boards (ASWB) and its contracted examination service. The examination is offered six (6) days a week at various locations and times. If you did not take an examination to receive licensure/certification in another state you will be required to take the ASWB examination of the appropriate licensure level before you will be licensed as a of social work in the State of Indiana. If applicants are seeking approval for the exam, applications for the licensure must be complete prior to the Board registering applicants with examination service. Once approval is granted, information will be emailed to the applicant explaining the registration and scheduling process for the examination. It is the responsibility of the applicant to register for and schedule the examination. An applicant who has been approved by the Board to take the examination must take the examination within one (1) year from the date of the initial Board approval. If the applicant does not take the examination within one (1) year from the date of the initial Board approval, the approval will be invalid and the applicant must submit a new application and all required documentation must be resubmitted. Testing Accommodation Request: If you have a disability, which may require a special accommodation in taking the examination, please contact the ASWB for information. Official Score Report: Upon completion of the ASWB level examination, results will be released to our office. If you passed the examination, your social work license will be issued. If you failed the examination, you will receive notification from our office via email with instructions on how to re-apply to retake the examination. You may use current marriage and family therapy licensure/certification held in another state to exempt yourself from retaking the AMFTRB examination, provided you have already successfully passed the AMFTRB examination or a substantially equivalent examination that also tests clinical skills and knowledge. Substantially equivalent as used in this manner is up to the Board's discretion. The Board has adopted the National Board for Certified Counselors (NBCC) examinations. The Board will accept the National Counselor Examination (NCE) to obtain the mental health associate counselor license and the National Clinical Mental Health Counselor Examination (NCMHCE) to obtain a mental health counselor license. If you are currently licensed as a mental health counselor in another state your are not required to sit for the exam provided you took the NCMHCE examination in another state or an equivalent state constructed examination at the same level or higher. The National Counselor Examination (NCE) is not an acceptable examination for mental health counselor licensure. If you did not take an examination to receive licensure/certification in the another state at the same level or higher level, you will be required to take the NCMHCE before you will be licensed as a mental health counselor in the State of Indiana. An applicant who has been approved by the Board to take the examination must take the examination within one (1) calendar year from the date of the initial Board approval. If the applicant has not taken the examination within one (1) calendar year from the date of initial Board approval, the approval will be invalid and the applicant must submit a new application and all required documentation must be resubmitted. No extensions will be granted. Application fee of $50.00: Fee is paid online during the online application process. All application fees are nonrefundable.

Required Documentation: Be prepared to provide the followiing with your online application:
Mission Statement: Applicants must provide a brief statement of their goals of the type of CE they will be presenting for the profession.
Presenter Information: Application must provide a list of names and credentials of all presenters. Please submit a copy of their Curriculum Vitae.
Programs Offered: Applicants must provide a list or brochure of the courses that will be offered as a CE Provider. This must include the names of the courses and where they will be held. If they are being held virtual, it must be indicated and if the course will have an exam at the end of the course.
Sample Course: Please provide a copy sample of one of the courses that will be presented, or that you have presented previously.
Sample Course Evaluation: Please provide a copy of the evaluation form you will be providing to professionals taking your course.

Sample Sign-in Sheet: Please provide a sample of the sign in sheet you will be using to keep track of the professionals attending your course(s). Your sheet at minimum should contain the following:

Frequently Asked Questions

If I hold an Indiana license where treatment for Addictions is included in the scope of practice, do I need an Addictions license?

To ensure that you are practicing addictions within the scope of practice, please review the statute and rules.

To obtain an addictions license in Indiana you must take and pass one of the national exams. You may find information on the exams in the Exams and Testing section of the Board page. If you wish to obtain approval to take one of the national exams, please review the application instructions for licensure by exam.

Please review the application instructions for the license level you are applying for. Information on who can supervise your are listed there.

You may still apply for licensure, but eligibility for licensure will depend upon the individual facts and circumstances of your criminal history. You will be required to present a brief statement and supporting documents of each incident. This information will be reviewed by the Board to determine your eligibility.

An LAC is a Licensed Addiction Counselor, which is a Bachelor education level licensure. An LCAC is a Licensed Clinical Addiction Counselor, which is a Masters education level licensure.

An Associate Addiction license is for individuals to obtain post degree experience and supervision. Associate licenses may only be renewed twice. Please review the statute and rules here.

You may apply for a Temporary Addictions Permit when you apply for approval to take the exam. Your Temporary Permit will last one year, and cannot be renewed.

Form E2 verifies your post-degree experience and employment. Form S2 verifies your post-degree supervision while you were employed. These forms must have matching time frames.

You may apply for a license and complete all documents for licensure. As of July 1, 2014 if you have not taken a national exam you must apply to take the exam.

To apply by reciprocity, you must hold a license or certificate in another state and have passed an equivalent national exam. If you only hold the CADAC credentials and not a license in another state, you will not qualify for reciprocity.

You must show that you have completed your educational requirements with your transcripts. You cannot use certificate training or Continuing Education seminars to meet the requirements.

If you hold an Indiana license and are physically located outside of Indiana, you must apply for a Telehealth Certificate through your online account before performing telehealth services to patients located within Indiana. Who can supervise me?

If you are working in Indiana and hold an Associate Mental Health Counselor license, please review the application instructions under the license type your are applying for.

Do I need supervision to apply for the LBSW or LSW license?

Graduates with a Bachelor of Social Work or Master of Social Work are required to obtain their license to practice. Supervision is not required to obtain the Bachelor or Master license, but post-licensure supervision of practice is required for those license levels.

If you are working in Indiana and hold a Licensed Social Worker (LSW) license, you must be supervised by an individual holding an Indiana Licensed Clinical Social Worker (LCSW) license .

You may still apply for licensure, but eligibility for licensure will depend upon the individual facts and circumstances of your criminal history.
